Key responsibilities for this position typically include:
- Assessing, planning, implementing, and evaluating patient care plans in a rehabilitation setting.
- Administering medications and treatments accurately and in accordance with hospital policies.
- Monitoring patient progress and collaborating with physicians, therapists, and other healthcare professionals.
- Educating patients and their families on post-discharge care and ongoing health management.
- Maintaining accurate and timely electronic medical records.
