As a Senior Living Physical Therapist, you will provide age-appropriate, direct patient care services tailored to adult and geriatric populations. Your daily work involves conducting comprehensive patient evaluations to determine rehabilitation potential, establishing individualized treatment plans, setting both short-term and long-term goals, and developing home exercise programs. You will also oversee treatments delivered by physical therapist assistants or technicians, participate in family and team conferences, and evaluate adaptive equipment needs.
Primary responsibilities include:
- Evaluating patients to identify needs, set goals, and create individualized treatment plans.
- Adhering strictly to established medical regimens and patient precautions such as THR, TKR, cardiac, and wound care.
- Utilizing safe and effective treatment techniques, including transfers, positioning, splinting, cryotherapy, ultrasound, and electrical stimulation.
- Maintaining equipment in a safe, proper manner, including completing inspection and maintenance logs and reporting faulty gear.
- Completing thorough discharge summaries for each patient on their final visit.
- Documenting all services accurately in accordance with department and facility standards.
Required qualifications for this position include:
- Current Physical Therapy licensure in the state of practice.
- Current CPR certification.
- A current driver’s license, vehicle insurance, and access to a dependable vehicle or reliable transportation.
- The ability to function professionally in any home environment regardless of the client’s age, race, background, or financial condition.
This role features a competitive annual total cash target pay ranging from $77,400 to $171,000, assuming full-time employment at full productivity. Compensation combines per-visit point rates and hourly pay for non-visit activities, depending on specific job duties and productivity metrics. Eligible team members may also receive a comprehensive benefits package, recognition programs, equity stock purchase options, and 401(k) contributions, subject to standard requirements.
