Optum is seeking a dedicated Weekday Per Diem Nurse Practitioner to join our Long Term Care team in San Antonio, TX (Bexar County). This position requires Monday through Friday weekday coverage and targets approximately 24 hours per month. External candidates may be eligible for a $3,500 sign-on bonus. As part of Optum Home & Community Care, you will work within our Senior Community Care product to provide vital care to patients in nursing homes, assisted living facilities, and senior housing.

In this role, you will play an essential part in transforming care delivery by offering innovative, personalized medical attention to patients in long-term care settings. Your daily responsibilities will include:

  • Conducting comprehensive clinical assessments for members
  • Managing members through acute medical situations or chronic condition exacerbations, including ordering and interpreting testing, diagnostics, and interventions
  • Overseeing members during short-term sub-acute rehabilitation episodes
  • Ensuring accurate, complete documentation, including appropriate ICD-10 conditions
  • Communicating and collaborating closely with an interdisciplinary care team
  • Conducting advanced illness and advanced care planning conversations
  • Providing meaningful counseling and education to patients and their caregivers

To qualify for this position, candidates must meet the following requirements:

  • Graduate of an accredited Master of Science in Nursing (MSN) or Doctor of Nursing Practice (DNP) program
  • Active and unrestricted nursing license in the state of Texas
  • 1+ years of clinical experience in practice
  • Proficient computer skills, including the ability to document medical information using written and electronic medical records
  • Ability to obtain post-hire or maintain a current, active DEA licensure and prescriptive authority
  • Ability to meet physical job requirements, such as moving a 30-pound bag in and out of a vehicle, navigating stairs and diverse dwelling conditions, and performing physical assessments
  • Valid driver’s license and reliable transportation to travel to member facilities (mileage is reimbursed)

Preferred qualifications include a strong understanding of geriatrics, chronic illness, advanced illness, and end-of-life care discussions. Pay for this role ranges from $43.22 to $77.21 per hour based on full-time equivalency, with actual compensation determined by factors such as education, local labor markets, and work experience. Eligible team members may also participate in comprehensive benefits, incentive programs, equity stock purchase, and 401(k) contributions subject to program rules.
