Nurse Practitioner or Physician Assistant – Oswego Onodaga Counties

Optum is currently seeking a dedicated Nurse Practitioner or Physician Assistant to join our team serving Oswego and Onodaga counties, based in Syracuse, NY. In this role, you will work within our Senior Community Care (SCC) program, providing vital clinical care to our highest-risk health plan members in facilities such as nursing homes, assisted living environments, and senior housing.

This position offers a balanced professional schedule designed to support your work-life balance, featuring no on-call hours, no weekends, and no required holidays. You will collaborate closely with an interdisciplinary care team to coordinate patient care and deliver life-changing support across care settings.

Primary responsibilities for this role include:

• Conducting comprehensive patient assessments and managing members through acute medical situations or chronic condition exacerbations, including diagnostic testing and interventions.
• Managing members during short-term sub-acute rehabilitation episodes.
• Ensuring accurate and complete clinical documentation, including ICD-10 conditions.
• Communicating and collaborating effectively with the interdisciplinary care team.
• Conducting advanced illness and advanced care planning conversations.
• Providing patients and caregivers with essential counseling and education.

To qualify for this position, candidates must meet the following requirements:

• An active and unrestricted license in the state where you reside, or the ability to obtain it by your start date.
• Active Nurse Practitioner or Physician Assistant certification through a national board, such as the American Academy of Nurse Practitioners or the American Nurses Credentialing Center, with certification in Family, Adult, Gerontology, or Adult-Gerontology Acute Care Nurse Practice.
• Current, active DEA licensure and prescriptive authority, or the ability to obtain it post-hire per state regulations.
• The ability to gain a collaborative practice agreement, if applicable in your state.
• The ability to meet physical job requirements, including moving a 30-pound bag in and out of a vehicle, navigating stairs and various dwelling conditions, and performing physical patient assessments.
• A valid driver’s license and access to reliable transportation.

Preferred qualifications that enhance your application include one or more years of clinical practice experience—particularly in long-term care settings—familiarity with complex behavioral and social patient needs, an understanding of geriatrics and chronic illness, experience with end-of-life discussions, and strong computer skills for electronic medical record documentation.

Compensation for this specialty generally ranges from $140,500 to $156,000, determined by factors such as local labor markets, education, and experience. Additionally, eligible individuals who have not previously participated in the program may qualify for a $40,000 student loan repayment or a $30,000 sign-on bonus.
