Optum Home & Community Care, part of the UnitedHealth Group family of businesses, is seeking a travel Nurse Practitioner or Physician Assistant to join our Senior Community Care (SCC) product. In this dynamic role, you will travel throughout the New York metro areas to provide integrated, longitudinal care to our highest-risk health plan members in facility settings such as nursing homes, assisted living, and senior housing.

This life-changing position allows you to work collaboratively with an interdisciplinary care team to improve patient access to care and achieve a healthy work-life balance, featuring no on-call shifts, no weekends, and no holidays required.

Key responsibilities for this role include:

  • Providing clinical coverage for the primary Advanced Practice Clinician (APC) during periods of PTO or short/long-term absences.
  • Managing members through acute medical situations or chronic condition exacerbations, including diagnostics and interventions.
  • Managing members during short-term, sub-acute rehabilitation episodes.
  • Conducting comprehensive assessments and effectively managing acute and chronic medical and behavioral conditions.
  • Ensuring accurate and complete documentation, including ICD-10 conditions.
  • Communicating and collaborating with the interdisciplinary care team.
  • Conducting advanced illness and advanced care planning conversations, and providing patient and caregiver counseling and education.

To be considered for this position, candidates must meet the following qualifications:

  • Active and unrestricted license or the ability to obtain by start date.
  • Certified Nurse Practitioner or Physician Assistant through a national board, with specific master’s degree and board certification requirements applicable to NPs (such as AANP, ANCC, FNP, or GNP) or PAs (NCCPA).
  • Current, active DEA licensure/prescriptive authority or ability to obtain post-hire per state regulations.
  • 1+ years of hands-on post-graduate experience, with a long-term care setting preferred.
  • Ability to complete physical requirements, including lifting a 30-pound bag in and out of a car, navigating stairs, and performing physical assessments.
  • Valid driver’s license and access to reliable transportation for travel throughout the New York metro areas.

We are proud to offer a competitive salary range of $125,000 to $175,000 annually based on full-time employment, along with a $40,000 student loan repayment or $40,000 sign-on bonus for individuals who have not previously participated in this program (eligibility rules and a 36-month service commitment apply).
