As an Occupational Therapist in Home Health, you will be responsible for the assessment and evaluation of patient care needs related to functional status, activities of daily living, fine motor coordination, home assessments, adaptive equipment, and other occupational therapy needs as defined by medical conditions.

Primary Responsibilities:

  • Provide service within the scope of practice as defined by state laws governing occupational therapy, in accordance with the plan of care, using evidence-based techniques, and in coordination with the health care team.
  • Evaluate the patient’s functional status, body systems status as required for CMS documentation, and occupational therapy needs, while consulting with physicians to develop the therapy plan of care.
  • Observe, record, and report patient responses to treatment and any changes in condition to the nurse supervisor and physician.
  • Conduct patient assessments evaluating the level of function by applying diagnostic and prognostic functional ability tests.

Required Qualifications:

  • Current Occupational Therapy licensure in the state of practice.
  • Current CPR certification.
  • Current driver’s license and vehicle insurance, access to a dependable vehicle, or reliable public transportation.
  • Ability to function in any home situation regardless of age, race, creed, color, sex, disability, or financial condition of the client.

Preferred Qualifications:

  • Demonstrated ability to manage multiple tasks simultaneously.
  • Demonstrated ability to work independently.
  • Good communication, writing, and organizational skills.

Compensation and Benefits:

  • The annual total cash target pay range is $71,700 to $158,400, assuming full-time employment at full productivity.
  • Compensation includes a per visit point rate of $41.37 to $91.38 and an hourly rate of $34.47 to $76.15 for non-visit activities such as orientation.
  • Total cash compensation is based on factors including local labor markets, education, and work experience, and may increase over time based on productivity and performance.
