Aster HR is currently recruiting for multiple experienced and collaborative Case Managers to join a new Families Together team on behalf of our client, SAGE Community Services. This is a full-time opportunity based in Sydney, specifically focusing on the Western Sydney regions of Parramatta and The Hills district.
In this vital role, you will work as a primary case manager within a dedicated child protection team. Your day-to-day focus will be on delivering high-quality, client-centred support to families under the Families Together contract, helping to drive positive outcomes and create a genuine local impact within the community.
Key responsibilities typically associated with this role include:
- Acting as the primary case manager for families, developing and implementing tailored support plans.
- Collaborating closely with internal team members, leadership, and external community services to ensure comprehensive care delivery.
- Maintaining accurate case notes, documentation, and reporting in alignment with organizational and child protection standards.
- Advocating for clients and empowering families to build resilience, safety, and long-term stability.
