Encompass Health Rehabilitation Hospital of Houston at The Medical Center is seeking a dedicated Clinical Dietitian / State Licensed Dietitian to join our team in the Fifth Ward/Frenchtown area of Houston. As a dietitian at Encompass Health, you will play a crucial role in promoting health and wellness through personalized nutrition plans for our rehabilitation patients. In this role, you will be responsible for assessing individual nutritional needs, developing and implementing targeted dietary plans, and providing valuable education on healthy eating habits and nutrition management.
Key responsibilities typically associated with this role include:
- Assessing patients’ comprehensive nutritional needs upon admission and throughout their stay
- Developing, implementing, and monitoring individualized dietary care plans in collaboration with the interdisciplinary healthcare team
- Instructing patients and their families on proper nutrition, therapeutic diets, and healthy eating habits
- Evaluating the effectiveness of nutrition interventions and modifying plans as necessary
- Documenting clinical nutrition information accurately in patient medical records
