Hartford HealthCare at Home is seeking a Hospice Admission Nurse (Visiting Registered Nurse) to join our team in the Avon, Hartford County area. As a Hospice Admission Nurse, you will be responsible for conducting start-of-care assessments for patients entering hospice services in their homes. This includes evaluating patient needs, developing individualized care plans in collaboration with the interdisciplinary team, and ensuring a smooth transition into hospice care. You will serve as the primary clinical point of contact for patients and families during the critical admission phase, providing education, emotional support, and guidance through the hospice process.
Key responsibilities include:
- Performing comprehensive nursing assessments at the patient’s home to determine eligibility and establish the plan of care
- Coordinating with physicians, social workers, chaplains, and other team members to ensure holistic, patient-centered care
- Educating patients and family members on hospice services, symptom management, and what to expect throughout the care journey
- Documenting assessments and care plans accurately and in a timely manner within the electronic health record
- Floating between the Central and Watertown regions as needed to support admission coverage
This position requires occasional weekends and occasional on-call availability. The salary for this role is $27,815 per year.
