We are seeking a dedicated Licensed Practical Nurse (LPN) to provide quality patient care and perform technical skilled care directly in the patient’s home environment.
Primary Responsibilities:
- Provide high-quality clinical services within the scope of practice defined by state nursing laws, in accordance with the established plan of care, and in coordination with the broader health care team while following infection control procedures.
- Document patient visits thoroughly and completely per regulatory and payer requirements using the electronic medical record and electronic visit verification programs as applicable.
- Actively participate in care coordination by reporting patient needs and condition changes promptly, attending case conferences, and communicating effectively with patients, families, physicians, clinicians, and agency staff.
Required Qualifications:
- Current and unrestricted LPN licensure in the state of practice.
- Current CPR certification.
- Current driver’s license, vehicle insurance, and dependable transportation or access to reliable public transportation.
- Ability to work flexible hours to meet identified client needs.
- Ability to function professionally in any home situation regardless of the client’s age, race, creed, color, sex, disability, or financial condition.
Preferred Qualifications:
- Previous home care experience.
- Ability to manage multiple tasks simultaneously and work independently.
- Good communication, writing, and organizational skills.
Compensation and Benefits:
- The hourly pay for this role ranges from $20.00 to $35.72 per hour based on full-time employment, with exact pay determined by factors such as local labor markets, education, work experience, and certifications.
