As a Licensed Practical Nurse in Home Health, you will be responsible for providing quality patient care and performing technical skilled care directly in the patient’s home environment. Your work will directly support our mission to help people live healthier lives and ensure that patients receive compassionate, professional medical support.
Primary Responsibilities:
- Provide high-quality clinical services within your scope of practice, following the established plan of care and coordinating with other members of the health care team while adhering to all infection control procedures.
- Document patient visits thoroughly and completely in compliance with regulatory and payer requirements using the electronic medical record and electronic visit verification program.
- Actively participate in the care coordination process by reporting patient needs and changes in condition, attending patient case conferences, and communicating effectively with patients, families, physicians, clinicians, and agency staff.
- Manage multiple tasks independently and efficiently while handling various home situations respectfully and professionally.
Required Qualifications:
- Current and unrestricted LPN licensure in the state of Washington.
- Current CPR certification.
- Current driver’s license and vehicle insurance, along with access to a dependable vehicle or reliable public transportation.
- Ability to work flexible hours as needed to meet client requirements.
- Ability to function effectively in any home setting regardless of the client’s age, race, creed, color, sex, disability, or financial condition.
Preferred Qualifications:
- Previous home care experience.
- Demonstrated ability to manage multiple tasks simultaneously and work independently.
- Strong communication, writing, and organizational skills.
Compensation and Benefits:
- The hourly pay for this full-time role ranges from $20.00 to $35.72 per hour, determined by factors such as local labor markets, education, work experience, and certifications.
