As an Occupational Therapist in Home Health, you will be responsible for the assessment and evaluation of patient care needs related to functional status, activities of daily living, fine motor coordination, home assessments, adaptive equipment, and other occupational therapy needs defined by medical conditions. You will play a vital role in delivering evidence-based care directly within the community.

Primary responsibilities include:

• Providing service within the scope of practice as defined by state laws governing occupational therapy, in accordance with the plan of care and in coordination with the broader health care team.
• Evaluating the patient’s functional status, the condition of all body systems required for CMS documentation, and specific occupational therapy needs, while consulting with physicians to develop the therapy plan of care.
• Observing, recording, and reporting the patient’s response to treatment and any changes in their condition to the nurse supervisor and physician.
• Conducting patient assessments to evaluate the level of function by applying diagnostic and prognostic functional ability tests.

Required qualifications for this role:

• Current Occupational Therapy licensure in the state of practice.
• Current CPR certification.
• Current driver’s license and vehicle insurance, along with access to a dependable vehicle or reliable transportation.
• The ability to function in any home situation regardless of age, race, creed, color, sex, disability, or financial condition of the client.

Preferred qualifications include:

• Demonstrated ability to manage multiple tasks simultaneously.
• Demonstrated ability to work independently.
• Good communication, writing, and organizational skills.

Compensation for this role is based on several factors, including local labor markets, education, work experience, and certifications. The hourly pay rate ranges from $34.23 to $61.15 per hour based on full-time employment, and we comply with all applicable minimum wage laws.
