As a Home Health Physical Therapist, you will play a vital role in assessing and evaluating patient care needs related to functional status, mobility, endurance, pain, and overall medical conditions. You will collaborate with physicians and the broader healthcare team to develop and implement effective treatment plans, perform targeted interventions, and continuously evaluate patient progress.

Primary responsibilities for this role include:

• Evaluating patient functional status and physical therapy needs to assist physicians in developing comprehensive care plans.
• Providing physical therapy services within your professional scope of practice, state laws, and agency policies.
• Observing and reporting patient responses to treatment and any condition changes to the supervising nurse and physician.
• Treating patients to relieve pain, develop and restore function, and maintain maximum independence.
• Instructing patients, families, caregivers, and team members regarding the plan of care, limitations, and goal progress.
• Completing timely visits, progress notes, and patient evaluations in accordance with organizational guidelines.

Required qualifications for this position include current Physical Therapy licensure in the state of practice, current CPR certification, a valid driver’s license with vehicle insurance, access to dependable transportation, and the ability to work effectively in diverse home environments.

Compensation for this full-time role ranges from $34.23 to $61.15 per hour, with exact pay determined by factors such as local labor markets, education, experience, and certifications.
