As a Home Health Physical Therapist, you will play a vital role in completing assessments and evaluations of patient care needs related to functional status, mobility, endurance, stamina, pain, and other medical conditions. You will help determine a comprehensive treatment plan, perform targeted interventions, and continuously evaluate patient progress.
Primary Responsibilities Include:
- Evaluating the patient’s functional status and physical therapy needs, and assisting the physician in developing the therapy plan of care.
- Providing services within the scope of practice defined by state laws and in coordination with the broader health care team.
- Observing and reporting the patient’s response to treatment and any condition changes to the supervising nurse or physician.
- Treating patients to relieve pain, develop and restore function, and maintain maximum functioning using acceptable practice standards.
- Instructing patients, families, caregivers, and team members regarding the plan of care, functional limitations, and goal progress.
- Visiting patients according to the Plan of Treatment, completing progress notes for each visit, and submitting documentation per LHC policy.
- Completing patient evaluations, establishing the physical therapy plan of care within state guidelines, and reporting outcomes and anticipated care frequency.
Required Qualifications:
- Current Physical Therapy licensure in the state of practice.
- Current CPR certification.
- Current driver’s license and vehicle insurance, along with access to a dependable vehicle or reliable transportation.
- Ability to function professionally in any home environment regardless of the client’s age, race, creed, color, sex, disability, or financial condition.
Compensation and Benefits:
- An hourly pay range from $34.23 to $61.15 per hour based on full-time employment, with pay determined by factors such as local labor markets, education, experience, and certifications.
- A $5,000 sign-on bonus available for external candidates.
- A comprehensive benefits package, including incentive and recognition programs, equity stock purchase, and 401(k) contributions, subject to eligibility requirements.
