As a Physical Therapist (PT) in Home Health, you will play a vital role in completing assessments and evaluations of patient care needs related to functional status, mobility, endurance, stamina, pain, and other medical conditions. Based on your evaluations, you will collaborate to determine and execute effective treatment plans, perform interventions aimed at enhancing patient well-being, and continuously evaluate patient progress.

Primary Responsibilities:

• Evaluates the patient’s functional status and physical therapy needs, assisting the physician in developing the therapy plan of care.
• Provides professional services within the scope of practice defined by state laws governing physical therapy, coordinating closely with other healthcare team members.
• Observes and reports the patient’s response to treatment and any condition changes to the supervising nurse and/or physician.
• Treats patients to relieve pain, develop and restore function, and maintain maximum functioning using acceptable practice standards.
• Instructs patients, families, caregivers, and team members regarding the plan of care, functional limitations, and progress toward goals.
• Visits patients according to the Plan of Treatment, completing and submitting timely progress notes per organizational policy.
• Completes all patient evaluations, establishes the PT plan of care within state guidelines, and reports outcomes, goals, and anticipated care frequency.

Required Qualifications:

• Current Physical Therapy licensure in the state of practice.
• Current CPR certification.
• Current driver’s license, vehicle insurance, and access to a dependable vehicle or reliable transportation.
• Ability to function professionally in diverse home environments regardless of client age, background, disability, or financial condition.

Compensation for this role ranges from $34.23 to $61.15 per hour based on full-time employment, with exact pay determined by factors such as local labor markets, education, and experience.
